I just got mine today. They left me a voice mail weeks ago asking for my model etc. I didn't call them back either and still received the widescreen disk. 
However the case was fucking wrecked (was delivered by UPS)

I got mine yesterday through UPS. It is the full DVD with the case and cover. I did not expect that. I have not had a chance to watch it yet to see if it does the same thing.

Got mine (Chicago). Didnt watch it yet, yet the disc looks fine. I actually never noticed any issues on my original ANH, yet got the replacement just in case. Will compare them this weekend.

they fixed my probs. I had some audio and video skips when the empire destroys leia's home planet. And when obi won senses a disturbance in the force. It plays fine now.
Although one of the tabs was broken that holds the dvd, luckily it didn;t come loose.

Got mine yesterday...box was kind of smashed, but the disc seems to be in okay condition. Haven't had a chance to watch it yet, though.
It came UPS wrapped in that white on the outside/metalic on the inside plastic-like rainproof wrap they use. Guess FOX is too cheap to use BOXES!

I got mine last night via UPS. Seems like a waste to send a replacement via UPS, but whatever. I hadn't watched/noticed the problems with the first, but cheers to fox for fixing this and making it relatively easy on the customer.
