My copy from Walmart did not have a rebate form. I just emailed Buena Vista and they sent me one.

What email address did you use to email them? I need a rebate form...I called BHVE and requested two and they just sent one

Can anyone (from Canada) confirm that the copies of The Rock sold at Future Shop and Best Buy (with the additional UPC sticker on the back, on top of the wrapping covering the original UPC, and no sticker on the front of the packaging advertisting the $10 mail-in-rebate) actually contain the rebate form inside the case?

You have to email them and request the form. Their online email form is linked to that page.

It did say if you own any previous dvd release of the Rock or Con Air, I would think so.
