Done.
If I can comment on something, as someone who studied marketing and does it professionally:
I think the questions don't seem to be too relevant to compulsive behavior. While I realize I am linking you to an astrology site, I did a 10-second search on Google and this was the first result that seemed applicable:
Italics are mine. The questions aren't perfect, but could address the topic.

Signs of Compulsive Shopping
A tendency to shop and spend large amounts of money during times of depression or emotional distress. (How do you feel after buying a DVD? Do you look forward to going on your weekly DVD buying trip?)
Buying excessive amounts of items that never get used, especially if they already possess these items at home. Compulsive shoppers may buy large amounts of clothing for example, when they have an excess of clothes at home that have never even been worn. ("How many unwatched DVDs do you have?")
Unused items are frequently disposed of, sometimes even in the original packaging or with the tags still attached. (Who throws a DVD out?)
Lying about shopping or the amount of money spent. ("If applicable, does your SO know how much you've spent on DVDs this year?" or
"How closely have you calculated your DVD expenditures?
A. To the penny
B. To the dollar
C. Within $50.
D. I have no idea how much I spent"
Running up large amounts of debt or buying unnecessary items instead of paying bills. ("Have you ever gone into a store without the intention of buying a DVD and decided to buy the DVD instead of the item you went to get in the first place?")
Borrowing or stealing money to shop. ("Have you ever stolen a DVD from a store or friend?")
Shopping or money spent interferes with work or relationships. ("Do you feel work gets in the way of your DVD marathons?")

Some of these are exaggerated, but you get the idea.

I took it too. Didn't know what to put for "How often do you shop?" so I put weekly. The truth is that there are close to 20 CD and DVD stores within a couple of blocks of my apartment, and I typically walk into one or two a day after work.

Question 6 could use an option for mass-market general retailers such as Wal-Mart, Target, or the like. Those places sell more DVDs than the other options. An option for used DVDs could also be in there.
The Bus, about your suggestion for questions: "Buying excessive amounts of items that never get used, especially if they already possess these items at home." How many times have you bought a DVD you already have without realizing it?

Turns out I didn't have it.

Once again....THANK YOU SO MUCH EVERYONE
You guys seriously know how to be there for a friend in need. I cannot believe how many survey reponses I got...and you wrote very interesting things in the fill in parts that I will probably quote. By the way, "The Bus," I really appreciate your feedback and I want to note that coming into launching this survey, I knew that there would be a ton of things that I would forget to do to it, or that someone would tell me could have been better a different way. Due to time constraints I really didn't have time to get extra opinions..but I might use some of the posts I have seen here for concepts as to WHY people go out to buy DVDs. I realize that that is one of the purposes of my assignment, but I figured that by figuring out general information, a picture can paint itself as well.
Thanks everybody again!
