Christ... there's been a lot, actually. Most of them are more lighthearted stuff like platformers / action games. Some maybe because the difficulty is rather demanding, but more often because the newest Metal Gear or Halo or something came along and diverted my attention.
However, one that I always feel bad about is Xenogears. I believe my last save is at the final boss, yet I never got around to beating him. Not sure why. I think it might've been one of those situations where I decided I needed to do some leveling up, etc and figured I'd come back to it, but never did. What sucks is I still spent 80 hours on the damn game, although if I went back now and beat the game, the ending would mean fuck-all to me since I don't even remember hardly anything about the game...

Phantasy Star II. I think you had to go more or less through a whole dungeon fighting random battles to get to the last boss, and then he'd kill me fairly quickly (or maybe in some way I just couldn't figure out--it was a long time ago so hard to remember) and after a half dozen tries or so I just wasn't up for it anymore.

Metroid Prime 2, I'm still in the process of finishing it. I've gotta start Beyond Good and Evil over again, cause I stopped playing it for whatever reason a long time ago...

There were plenty of NES/SNES games I never finished (the original TMNT, Battletoads, etc) but every PS1, PS2, Xbox, and 360 game I have played, I am pretty sure I have finished them all. As I got older, I developed kind of this attitude where I can not stand having unfinished projects just laying around that I will finish "someday." I always try to finish what I start, and I am really anal about it. I can't think of a game I have played in the last 3 generations that I never finished because of it. Even games I didn't like, I just "had" to finish anyway.

I know of more than a few in my collection. I'm currently playing Star Ocean 3, (for about six month now. Gives you an indicator of how long it takes me.) so I guess that counts.
The PS2 I can think of are GTAIII (despite beating VC and SA), Driver 3, Shadow Hearts and a bunch with stuff to unlock.
My PS1 has a plethora of games I got bored with and never finished.

The fourth game in the original .hack// series. Loved the story, but I took a break from playing the fourth game due to a rough semester of classes and when I started playing again I couldn't get back into it.

Actually there are two, Star Wars Jedi Outcast (PC) and Doom for Xbox. I was in the middle of SW:Jedi Outcast when the Xbox craze hit so that took a back seat. I got Doom on sale after it had been out awhile but Halo was still hot. I took a long break from gaming then the 360 hit.

Same here, no matter how hard I tried I couldnt get into that game. I really wanted to complete it because I heard the story was the best part, but I always got distracted with another game
