If we count shows that come back after years off the air there would likely be a lot more too. For instance Hugh Beaumont's character from Leave it to Beaver was "killed off" between series due to the death of the actor.

No no, she died in a fire at the family house. They even showed the fire going on and the kids screaming "MOMMMMM!!!!!!" at the flames. Very awkwardly morose.

Bart Simpsons teacher, Edna Krabappel, died off-screen, owing to the death of voice actress Marcia Wallace.

The Valerie Hogan character was written out of the show as having died in a car accident, and NBC explained that the series would take on difficult issues facing the family during the grieving process.
But later on there was a fire:
The season's third episode, "Burned Out" (aired October 5, 1987), helped better explain the family's grief following the death of Valerie's character. In that episode, a lamp stored in the attic develops a short circuit, sparking a fire that badly damages the house. Many keepsakes and mementos of the family's were destroyed in the attic and second floor, including a charred framed photo of Valerie. David found the photo in his room while the fire damage was being inspected, and immediately broke down in tears. Sandy came into the room to comfort him, and as the scene changes, it is presumed that the two shared their grief for Valerie. The Hogans, meanwhile, stay with the Pooles while their home is repaired. The episode had a commercial tie-in with the McDonalds Corporation, which financed the expenses accrued in damaging the set for the fire. As a sponsor that evening, McDonald's commercials aired promoting fire safety.
I have no idea where I came up with the candy rack falling on somebody, now I'm off to google that until I have an answer.

Ward Bond (Maj. Seth Adams) died during the 3rd season of Wagon Train. He was replaced by John Macintyre, but his disappearance was never explained.

Another one: Jon Erik Hexum, who was on Cover Up, died during production of the series when he shot himself in the head with a blank. His character was sent away on a special mission and kiled off-screen.

When Jack Soo died during the filming of Barney Miller, they did a special episode where the cast sat on the set, dressed like they would, but looking to the audience and talking about memorable scenes with Jack. Was quite touching.

Whose careers should have died offscreen... the writers of Heroes.

Gant died onscreen. A patient with severe wounds to is brought in and Benton has Carter text Gant to help and the pager on the patient goes off. His wounds were so severe they didn't recognize him(given that, I have no idea if it was actually Epps on the table. Maybe you're thinking of what led him to the gurney... jumping/falling onto the 'L' tracks, that was never shown.

Something similar was done after TR (TJ?) Knight left "Grey's Anatomy". His character was hit by a bus (while saving the life of another) and no one recognized his mangled body when it was brought into the ER. When Knight's character ("George") was called in, his beeper went off and his co-workers realized the patient was him. I hope I'm remembering that right.




Both explanations felt off to me so I looked it up as well.
