Toy Story. I think the fuckers running the movie were trying to take into account a theater full of screaming, crying, rugrats...They overcompensated!

First 3 that came to mind:
Sicario from this year. The music (The Beast) as they enter Juarez was loud as hell and suitably cool. Same thing for the shoot outs.
Drive. Very quiet movie, but saw it in a theater that had a highly souped up system and the action scenes had the effects and music turned to full, and it was really great.
Terminator 2. Saw this first screening opening day in a massive cinema that had possibly the best sound system i have ever experienced. That opening scene alone will be a cinematic memory that will stay with me forever. Just awesome.

I have two, and interestingly, both were sneak previews, as if they were still tinkering with the levels. The first was Aliens, at an employee preview in Newport Beach, California (the big Newport theater). That shot of the drop ship descending/screaming toward the planet was legitimately deafening. I think my ears still ring a little from that. The second was The Crow, at a horror convention preview in Arizona.

Also, the first Jurassic Park - that was back in the days when they cranked up the volume - it was so friggin' loud in the theater - loved it (being very young at the time made it all the more great as well).

Jurassic Park on opening day at one of the Century Domes. so loud.
A year or so ago i took the family to a movie and the sound got messed up at the start. It sounded like they were sending digital data into an analog input. it was terrible digital noise at "you're going to blow the speakers out" volume. Everyone had their fingers in their ears. it took them way too long to fix the problem too; like minutes too long

Fast & Furious 6. It got to the point during a couple of the action sequences, many of the people in the theaters started to cover their ears.
Twister was another one. That is the only movie I've ever seen that the theater had put up flyers at the ticket window to warn people that the movie was intentionally loud.

In 1977 I wanted to see Star Wars a second time, but my wife wanted to see Roller Coaster. I had no interest in RC as I thought the idea was lame and that it was playing in Sensurround was evidence to support my contention. I had seen Earthquake in Sensurround so I had experience to base my opinion on. She wasn't interested in seeing SW again so we each went to see what we wanted. The theater had three auditoriums, one large with two smaller ones on each side, and SW was playing the large screen.
Every time Sensurround kicked in I could hear it.
Insult to injury… afterwards my wife said she should have gone to see SW again.

Jurassic World. It was my second viewing, and it was the Regal Experience (tm) showing which was the only time I could go in. It was 2D but cost same as 3D. But WOW, the sound was BLASTING. Like OMG! *covers ears* It was awesome.
Other than that, for a "regular" sound, I think loudest would be Transformers 2, which was the last Trasnformers movie I watched.
