CD Japan is very reliable, but unlike Amazon, you pay the full retail Japanese price, and the shipping and handling are a bit steeper (but if Amazon is sold out, those points are moot anyway). At least I can firmly state that they are reputable, as I've ordered from them several times and have had flawless service.

HMV is one of the largest record store chains in the world. They're not a fly-by-night, although I've never ordered from them online, from Japan or otherwise.

With regards to your questions the easiest would be to check their online help. It's very comprehensive.
They do indeed ship to the US.

dang. sat on this one too long, and missed out on the amazon.co.jp pre-orders. had to go to hmv.co.jp and pay the extra shipping. the difference a day can make, sheesh!!

I highly doubt that we'll get something this good over here in Region 1. The only time we seem to get big promotional items with DVDs seems to be with anime releases (dolls, t-shirts, dog tags) and that's only because the market for anime is such a niche one. A movie this big will have a huge market for one and you know the same Joe Six packs who complain about widescreen are going to take one look at the $70 price tag and think Miramax is crazy. Nah, they'll probably only release a box set that has just the box and the DVDs. Think of the contraversy it would start if soccer moms picked up this DVD for themselves or their teenagers and found a tiny sword inside. True whoever's showing this would be treated to a bloody mess of a movie, but still you know that society would find a way to pick on the sword rather then the movie.

Since the set was going for around $90-100 USD for the pre-order, $155 on eBay really isn't that much of a gouge, especially considering that it's a limited edition of 30,000.
