FYI I got my rebate for Transformers using the HD DVD proof of purchase

Well, they cut the check on the 23rd, so I assume they mailed it that same day. I got it on the 25th. I'd give it 'til tomorrow or Monday.

What needs to be cut out and sent in for these? In other words, how badly do I need to mutilate the BD and DVD packaging?

You have to cut out a DVD proof of purchase and Blu-Ray proof of purchase.

Unfortunately, the proof of purchase on Transformers was on the plastic slip cover. It just didn't seem worth it to mutilate the box.

Yeah--I used a knife to cut it. I was upgrading from the HD-DVD, so I had to do that twice. But seeing as I got the Transformers blu for $9.99 at Amazon on Black Friday, it was well worth cutting it up to get it for free.

For the Blurays and HD-DVDs, it's a rectangle about 2 inches wide by 1/4 inch tall that is right above the UPC code on the back of the case. You could just pull out the insert and cut it out--don't need to hack away to get to it--it's right at the top. But not perforated tabs or a separate insert inside (that would have been nice.)

Well still no check. I have a feeling it got lost in the mail (not the first time with our post office). I also have an even bigger feeling that it won't get me anywhere/take forever to request a new one

Will photo copies of the UPCs be accepted or do I have to cut out the actual UPCs. I really don't want to mutilate my slipcover.

Bummer... that was my first time getting a rebate back from Paramount, so don't know how good they are with re-issuing. I'd at least try, though, seeing as how you had to cut up your cases.

I think if you look on the little slip you have to fill out and mail-in, it says photocopies are not allowed. I remember that I scoured it b/c I had my qualms about cutting up the covers as well. For $10, though, I thought it was worth it.

877-322-8355 is the number I've used.

I mailed in the forms and proof of purchases for Event Horizon and The Truman Show back in January. In February, I got two postcards saying my rebates were denied because I didn't include the proof of purchases. I called them then and was told it was an error on their part and that my checks should arrive within 10 days. After not hearing or receiving anything for two months, I called back today. According to the lady I talked to at the fulfillment place, Paramount made a big mistake and denied a lot of claims incorrectly. She said they claimed that they'd take care of the problem directly, but that it seems they haven't. Regardless, she said she was processing my rebates today, so we'll see.

I know this is somewhat off topic (since it's not Paramount) but does any one have the contact information for Disney rebates? I sent in my $10 BD rebate for Narnia around the same time. I found this site but I think it may be for another promotion.

i bought all 3 re-released Godfather flicks on Black Friday but then best buy had the trilogy for 49.99 so i cut out the godfather 1s proof of purchase ...

Sorry to quote myself, but I've had no luck getting a form. Does anyone have an extra they aren't going to use? I would certainly reimburse shipping and/or trade an unused one from the other promotion (Godfather/Sweeney Todd/Old School/etc). A PM would be appreciated.

I contacted CMS rebate center about the status of my Transformers rebate and they say they don't have a record of me sending it in. They said they need to know where I sent it to. I did make a copy of the form I sent in but unfortunately the address where I sent it in not visible due to the UPC codes. If any knows the address that we send the rebates to it would be greatly appreciated. Thanks in advance.

I know I'm bumping up an old thread, but I was curious about the Transformers rebate. I didn't realize you could send in the HD DVD proof of purchase for it, so I never bothered. I've been wanting to get the Blu-ray, as I'm trying to sell my HD DVD player (as I need the money), but then I would be left without Transformers (and also a couple of others that I would like to keep that I haven't picked up on Blu-ray, yet). Is this rebate still available? I just saw an old post on another site about Best Buy having a deal on the Blu-ray a few weeks ago and that someone was still able to get the rebate form and it was good until the end of this year. Is that true? Can the rebate forms still be found? If so, I might look for a deal on this and hope to pick it up before I sell off my HD DVDs (if I ever do )

Not sure when this deal ends--I thought there was a phone number you could call to get the rebate forms sent to you.
Note you will need to cut the POP from the outer plastic slipcover of both the HD-DVD and the blu-ray using a knife (it's not like the others where you can just cut out of the insert with scissors). I did this last year when the blu was only $9.99 on amazon black friday, so it was just the cost of a stamp (and having to cut up the cases).
