Have you ever broken a DVD trying to pry it out of its case?

I find a lot of DVDs that have these types of packaging are either hit-or-miss. Some I can get the disc out no problem, but others I have to work to get it out. I can't say i've ever cracked a DVD from taking it out of one of these cases tho.
I'll tell you what type of case I really love, it's one of those that have about 6 or so little tabs that you push down on to release the disc. They keep the disc super secure on the hub and aren't hard to remove at all.

Yes. Cracked the inside ring of Gladiator Disc 1. It will not play. Sucks to have a 2 disc set with one bad disc.
I had a lot of trouble with a Sopranos disc too, but it finally popped off.

Hmm. Broke a CD by keeping it in my pocket, once.
Haven't personally broken a DVD, but I found someone else's f***-up:
Borrowed a kids DVD from the library, opened it for a quick check as usual, seemed ok. Kids going crazy, MUST see this stupid DVD, gotta love toddlers. Get home, go to take out disc, it's a sliced pie of pieces. Didn't recognize that with my quick glance because the label was holding it together so well you couldn't really tell without careful examination...or by turning it over, of course. Needless to say, that was a BAD night with the kids. And I now give a better check on the borrowed/rental discs.
