Also off the top of my head.
Sliders Killing off the Professor. Sliders was never the same after that.
Prison Break Not ending the show at the end of season 2.

Plus: Sex and the City; I was never a big fan of the Russian story arch - but it's still my favorite show ever. That '70s Show; missed Eric and Kelso in the final season. And the countless examples of shows that were canceled before they could properly end or allowed to continue until they were deader than dead.

Rechargable batteries, records for the homemade record player and change of clothing for the Howells and Ginger. It was almost unbelievable.

Angel- the vast majority of Season 4
Buffy- equating magic use with drug addiction in S6, anything to do with Riley
The Office (US)- trying to make Jim and Pam's relationship look rocky when it just isn't
Veronica Mars - Pyz aka the Riley of V Mars; switching to standalones and mini-arcs in the third season (they had to, but still)
Doctor Who- low budgets and horribly camp acting

One flaw in Seinfeld that always bugged me: the inconsistency in the way Elaine was written.
For example, in the episode where she is getting the video recommendations from thy mysterious video clerk (who turns out to be a teenager) she is all into the art type movies, and rejects popcorn movies. Then in another episode, she despises the arty type movies and only wants to see the dumb, broad comedy movies, like the one that had the poster with the family in a grocery store bag. She said she wanted to see it to see how the family got in the bag.
I don't know...it's just one of those things that always bugged me in an otherwise great series.

Chuck relies too much on pop culture references in my opinion. Sure, it's funny to me now (and maybe always), but not to other people and certainly won't be to anyone who is very young now or hasn't been born yet.
Lost is too focused on the romantic entanglements of Jack, Kate, Sawyer, and Juliet. They already have plenty of those types of relationships on the show that manage to carry true emotional weight.

I would go with the Office. A couple things bug me that I have a hard time getting past. If it is a documentary about the inner workings of a regional paper company, is the footage ever going to air? It's been five years and this hasn't been brought up on the show yet. Second, in some episodes there are way too many cameras when staffers leave the office. I know it might be nit picking but I see those as flaws.

Journeyman: In the pilot, the chemistry between the main character and his wife was extremely bad. Sure got better, but it made the pilot very lackluster.

Rescue Me: Last season, where Dennis Leary Mary Sue'd Tommy into having sex with every attractive they could cast.

Family Ties. Whenever there was a conflict between two chracters, the episode ended with a minor variation of the following: Members of the family are gathered in the kitchen or living room with one of the members of the conflicted pair. There's a knock on the door. It's the other person involved in the conflict. The other family members say, "We'll leave you two alone". The conflicted pair has a heartfelt talk about the offending issue and makes amends. Credits. Applause.
As much as I love "Family Ties", the almost weekly repetition of that scenario was always predictable, facile, and tiresome.
