The only problems I have ever had with my DirecTivo is a few times when a show changes at the last minute, like going from a 9:00 to a 9:01 start time and the machine doesn't get a chance to "re-index" before the airing so it doesn't catch the "newly added" episode. That has only happened to me about 2 times now.
I check to see why it wasn't recorded and it said the show was "no longer in the guide", but what happened was the guide changed before a re-index caught the updated listing.
I think the Tivo re-indexes a few times a day or daily so a last-minute change can occasionally cause this type of hiccup.

I usually have the opposite problem... a show on cable is flagged 'new' for every airing. It once recorded the same episode of one show twelve times in one week. It filled my DVR up and bumped off some shows I actually wanted to want. Ever since then I don't screw with the season pass deals. Besides, with changing schedules, shows running over 5 minutes, etc. I sometimes have to use the other DVR for shows. It's easier just to plan that out myself instead of worrying about priorities and whatnot.

I've had a DirecTivo for almost two years now, and it has never missed a first run show - although it has recorded a number of episodes that it mistook for being first run.
