The "double-o" means that particular agent has a license to kill when necessary to accomplish their given their assignment.

So there's an agent 07 that can't kill without permission, and Bond probably gives him shit in the MI-5 cafeteria.

Also, regarding "double-o" status, in one of the original Fleming Bond novels, it was mentioned that the "double-o" meant they had killed in the line of duty, which make the "double-o" more of an honorary designation for agents who had killed for their cause, kind of like certain patches guys in biker gangs wear -- like the "Men of Mayhem" patch in Sons of Anarchy, but there are real-world examples in outlaw biker culture.

But it's always been my understanding that the "double-o" designation is a literal license to kill, and agents who have that number are the elitist of the elite and used in black ops situations or extremely difficult or sensitive missions.
