Well, the one that went over my head when I was a little kid watching reruns of Bewitched was the Dick York/ Dick Sargent switch. They looked exactly the same.
A similar (yet opposite) situation with me was when I watched "The Patty Duke Show" on Nick at Night. I didn't realize that Patty Duke played two roles (like I said, when you're a kid you're retarded) and furthermore I would have fiercely maintained at the time that I could physically tell the difference between the two of them.

IMDB ratings are similar, and given that recent movies are getting higher ratings as more people use IMDB, I think that one could argue that more people prefer Manhunter over Red Dragon.

In the Batman movies not only did different actors play the same character but the character changed races. Billy Dee Williams and Tommy Lee Jones as Harvey Dent.
Lana Lang in the Superman movies from i and III were played by different actresses.
If the new Star Trek movie gets made Kirk and Spock will be played by new, younger actors.
