How are cities/areas tiered for theatrical distribution?

How are cities/areas tiered for theatrical distribution?
Or more specifically how does Hollywood determine what the Top cities are for commercial distribution/limited release?
New York and Los Angeles are obviousily the top two. From what I have read if a movie doesn't do well in New York is declared commercially dead.
what are the remaining cities?
I gather it's
Chicago
San Francisco
Boston
Washington DC is somewhere in there, but its definately not in the Top 5
also where are the cities/theatres that cater to test screenings?
