Irreversible was crazy disturbing. It still haunts me.

Others that come to mind:

Gummo
Clean Shaven
Blue Velvet
HATED: G.G. Allen & the Murder Junkies
and of course the"reality" DVDs such as Banned from Television and Bumfights. Pure Depravity.

Although not out on DVD yet, I remember The Addiction with Lili Taylor as particularly ill.

I would agree, almost forgot about Kids and Leaving Las Vegas. Kids was a real shot in the arm about todays youth. Vegas starts on a downnote and just keeps going down from there.

Seeing American Beauty a week before my wedding was disturbing. How about Faces of Death? Does that count? I only saw the first two back in the 80's when I was a teenager and I remember that being very disturbing.

I agree about Irrversible. There is one scene that is so disturbing I cringed just thinking about it.

The most disturbing thing I've seen lately was Bumfights.

Other than that:
Natural Born Killers
Requiem For A Dream
Frailty

And when I was younger I always thought A Clockwork Orange was disturbing, but I don't now.

Also Faces Of Death on VHS which I haven't seen in forever, but I remember not being able to watch it all more than once.

A lot of Faces of Death is fake so it doesn't bother me much, I think Salo takes the cake here.

I'd have to say Men Behind The Sun, but Salo certainly is a close 2nd.

American History X really bothered me when I saw it. Haven't been able to purchase it because I don't know if I want to go through the feelings again.

I’m not disturbed easily, so most of what has been listed didn’t bother me one bit. Most of what has been listed I loved and enjoy viewing over and over again. Black Hawk Down and We Were Soldiers made me stop and think about my time in the service during the Gulf War, but not disturbing.

Movies that did make me feel sick to my stomach were Bio Dome and Dude Where’s My Car. They made me sick, gave me a headache, and lowered my IQ to -100.
