Sure .. the endings in Sixth Sense and Usual Suspects are well known.
But there's a little known gem of a movie, Lawn Dogs, which ..
Spoiler:
has a surreal and breath-taking twist at the end that allows the girl's friend to get away. It is all the more surprising because the sudden fantastical element wasn't hinted at all in the rest of the movie.

People have mentioned some of the more obvious ones, but one of the ones that really “got me” was John Carpenter’s The Thing, only because I didn’t catch it until a second viewing.

Watch the last few seconds of the movie, it is totaly obvious

Russell's breath is so exagerated it looks like he's smoking a cigar. Considering they are sitting side by side in the scene it's obvious it was very intentional

FWIW, I'm pretty sure that either on the DVD commentary, in interview(s) or both, Carpenter mentioned that he's heard this theory, denied that it was intended and stated that it was simply a product of the harsh lighting and camera angles.

My pick is Don't Look Now.

The figure in the red raincoat, turning to reveal itself to be the deformed and homicidal dwarf, caused me to jump out of my seat, literally. I didn't expect it to be his dead daughter, as the Sutherland character did, but the shock of what it was got me.

Two films that gave me that "Wow they ended it that way" were Das Boot talk about irony in war and To Live and Die in LA the ending was so sudden it caught me by suprise it was a definite HolyS**t moment.
