Nope. Although recently I forgot what Bond movies have and have not been released when I was looking at the single releases at Target.

Never, but I've come close a few times. Not even because I thought, "Hey, I might already have this..." but rather, "Well... I'll hold off. Maybe it'll get cheaper..." and then a week or two later, when browsing my collection, there it is... purchased many months ago. 
So, now I've installed the blu-ray.com app. I'm going to try it out this weekend. Can you just scan the barcodes or do you have to enter each title manually?

I've never done it but I've come close a few times where I've had to stop in the store and think real hard about whether I own a Blu of that movie yet or not.
I was looking over my collection the other day and was surprised that I own The DaVinci Code. When the hell did I buy that?!

yes, I thought I had loaned out a Kill Bill 1 & 2 Double Feature, so I bought another one for $15, but soon found it had simply slid to the back of the shelf behind other discs.

I just did it last week, could've sworn I had'nt upgraded my dvd of The Good, The Bad, and the Ugly to blu, so I bought it, took it home and there it was on my shelf...

No but sometimes I have a hard time remembering what I have, after DVD and HD-DVD purchases.

. I bought a few hundred at Blockbuster for .99 during Memorial Day '11 and have accidentally rebought a couple of those, but nothing I else I can remember.

I haven't bought any duplicate blus yet. I've bought a few duplicate DVDs (probably half a dozen or so over the past decade). And recently I bought a bluray of a title that I didn't realize I had on DVD.
Not bad given the fact that I'm still just relying on my memory.

I bought a copy of "Spinal Tap" last year, brought it home and stuck it on the shelf...and then stopped when I realized I was putting it beside a copy of..."Spinal Tap." That was the only time though and fortunately I hadn't opened it yet.

Not yet. Not quite the same, but I've recorded so many films on my DVR during free preview weekends and such that I've clicked on something only to see a message about "it already existing" in my recordings.

well...i did it again. bought Swingers for $5.00 @ Target and sure enough...already in my collection. i swear i am not this bad!

It has happened 4ish times (3 DVDs 1 BR)
Most recently, today, I rebought the Shriek Show Jungle Girls 3 pack. It was in 3 individual cases in a slip cover the first time I bought it, now all three are in 1 case. But it was only $4 so I think I will make it a gift.
My one Blu-Ray blunder was I Love You Man. Bought it the last U&S sale and the U&S sale before that! Still in the 'to watch' pile is why it hadn't clicked in my brain. For $5 I will gift it as well.
Document Of The Dead individual release forgetting it was in the Dawn Of The Dead Ultimate Edition. Sort of justifiable as it includes a commentary not present in the Ultimate edition.
And the first time was a Stan Lee Marvel thing not remembering it was included in the Limited Edition Spiderman box set.

Doesn't really happen. I keep a very up to date DVDProfiler set up. I also keep a txt file of my collection, noting what occasion I got the dvd or blu-ray for (birthday, christmas, etc.) The only time i buy a second copy of a movie is if I have the movie and it's also included on a multi-movie pack. For instance, I had the Thunderbirds movie. I bought a 4 movie set that had Thunderbirds in it. No big deal. And I had the trilogy set of the Back To The Future movies, but I, knowingly, bought the special edition release of the first movie so I could get the rid footage and the Leslie Nielsen special.
