Missing Cover Slip for Miracle DVD

Got the Miracle DVD today and it didnt have the chapter list paper inside the DVD.

Anyone else have this problem?

Go to the "cover, case, and inserts" section and ask there. There's a reason for that forum.

Grrrr.... mine was missing too. I guess I gotta head back up to Best Buy to get a different one.
