Why does CH lose application / orders??

Why does CH lose application / orders??
I made the following order to fulfill my TV Club account -
Crash [Director's Cut pre-order] - $19.95
Mission Hill Complete Series - $22.95
Harry Potter and the Goblet of Fire SE - Free
Boogie Nights - Free
With webdvd and a BOGO code [807749], it came to $37.41 I believe. However, two days later, I find that the order has disappeared completely. I ordered it again today. This is the 2nd order I've had disappear. The first one was a large order online [10 DVDs]. I had to reorder over the phone, which took forever.
I hear a lot of people complaining about their orders or applications just plain disappearing from CH's records. Why is this? Is their website just poorly built, or are they selectively ignoring orders? I find it extremely annoying.
