Rhaegar makes the most sense. The most likely scenario is that Lyanna ran off voluntarily with Rhaegar because she didn't want to marry Robert Baratheon. Rhaegar was in a political marriage with Elia Martell and seemed to have genuine feelings for Lyanna.

Unless it's something out of left field like the Mad King raping and impregnating Lyanna, and Rhaegar hiding her away to protect her and the child she was going to have. (How would Robert or Rickard Stark have reacted?)

With Walder Frey's death, has every single male head of house that we've known of been assassinated/killed/died? Dunno what's up with Braavos though.

For anyone wondering about it, this is an extended scene that shows how Varys is able to travel around so quickly

Mace Tyrell died in the episode too.

The two that I can think of that have origins as far back as season one that we have met and are definitely still alive are Randyll Tarly, Sam's dad, and Yohn Royce, who's son died in the opening scenes beyond the wall in season one. He's the main noble character from the Vale who has interacted with Littlefinger.

I guess we just finally met Wyman Manderly too.

Most of the major ones I believe. There is still House Manderly, Edmere Tully had Riverrun but I guess they put him back in the cell, so who knows who is running that now. The Vale is run by Lord Robyn isn't it? House Stark should technically be currently run by Sansa, but they pledged to Jon Snow as "King of the North!!!"

I like analyzing things that happen on Game of Thrones, but I don't really care about figuring out how long it takes for people to get from one place to another, unless it really has a big impact on the plot.

Depends on how you qualify things. Technically Arryn has stayed the same because Jon was already dead at the beginning of the story and so Robin has always been the head from the start. The other major houses have all seen changes, but there are plenty of smaller ones who haven't; Seaworth, Reed, and Manderly for example.

Expand that out to which houses that have any consequence to the story that have not lost either a Lord or an heir or both, and you're down to House Tarly and (arguably) House Arryn. And Arryn is quite arguable indeed since we don't know much time passed between the scene when the younger Royce is killed and the day that the Night's Watch ranger is executed. Maybe it was only a day or two or maybe the ranger was on the run for a few weeks. We don't really know. Arryn might have conceivably still been drawing breath when the show started.

Am I missing any others (besides a few that are barely in the story)?

Question about Dorne (even though people love to shit on it): Does it have any traditions/practices that Daenerys would want them to stop doing? She's all about making things better, but it seems like Dorne itself is pretty liberal (from what I've seen on the show), so I assume they'd be cool with each other after the dust settles (if they get to that point).

Same question for wherever the Tyrells are located.

I would think Tarly doesn't really qualify, but if you are saying it does then Tarth, Baelish, and Selmy would fit too. All have only one character that impacts the story, but their leadership seemingly has not been changed in any way.

Now that Arya has killed off the Freys (at least the important ones), could Edmure be freed to rule Riverrun again?

For sure, but with only so many episodes left, we might never find out either. I'm hoping we do get something though. Tobias Menzies is a great actor, it would be nice to utilize him more.
