Well, it's obvious that that wasn't her head in the box. They probably kept her alive as a final bargaining chip against Michael.
This should help the ratings as she was a very sympathetic character that a lot of people seemed to like. I hope they bring that smokin' hot Latina back next year. The bad girl with the horribly ugly eyes can come back also, because she is a worthy adversary against the good guys.

PRISON BREAK’s Matt Olmstead Reveals the Truth about Sarah Tancredi

Happy Fools Day everyone! Sarah Wayne Callies is returning to PRISON BREAK. What’s that? It’s not April first? WTF?!?!?
No joke folks, executive producer Matt Olmstead just confirmed in an interview with TV Guide’s Super Scooper Michael Ausiello that the team at PRISON BREAK has entered the Guinness Book of World Record’s. For the biggest shark jump ever.
Apparently, Sara’s head wasn’t in the box. Let the Michael Ausiello interrogation begin.
But initially it was supposed to be her head. What changed?
Olmstead: What changed was…. once we realized that the emotional hook of Season 3 was going to be the death of Sara, when we didn’t get the actress to do it, as soon as we wrote it and shot it, we realized that there was actually a way she could still be alive.
Interesting ’story’ Mr. Olmstead. Here’s what we think Matt’s answer should have been:

Fake Matt Olmstead: What changed was…. we’ve simply run out of ideas. I mean Michael’s been in prison, out of prison and back in prison again. If it wasn’t clear enough to the show’s dwindling audience, PRISON BREAK is running on empty. This show had one solid year of story in it, maybe two… You tell me, what else were we supposed to do for a fourth season?
Back to the real interview, Ausiello asked:
What changed for her that she’s willing to come back now?
Olmstead: She and I spoke a couple of days ago at length about what we have come up with creativelty for her character…
Again, here’s what Matt’s answer should have been:

FOX is desperate to get this show to one hundred episodes. You know syndication, where the real money in television is made. So we backed up the Brinks Truck to her doorstep. No joke, Dominic [Purcell] drove it. I’ve posted photos on my Facebook.
So there you have it PRISON BREAK fans. The most unbelievable and implausible PRISON BREAK story-line since the time T-Bag suchered his own hand back on. But the big question remains, do you care? Will Sara’s return get you back on the bandwagon? Or is this the proverbial straw that broke the camel’s back? After-all, who really watched PRISON BREAK for the realism? Post away with your thoughts.

Pretty obvious explanation here. The show lost over 2M viewers in season 3. They probably pitched this to her and the network so that season 4 would get the green light.
Also, I believe now that the show is re-locating to Los Angeles, it will be more convenient for Ms. Callies and her family to work on the show as opposed to flying to Dallas and working in the heat for 10 months.

So I saw a new ad during The Simpsons tonight that I hadn't seen before.
It looks like they're taking the show in an entirely new direction which actually looks entertaining, but it's like they're turning it into The Mod Squad
oddly enough, this is probably the best way to keep the series going.
