It's the little things that really kill me:
- Maintaining continuity with all the lies Janitor has told before. (I especially like the "my wife only has pointer and thumb/pinky")
- Gloria clearing her throat right before she speaks into Dr. Cox' pen mic
- Keith in the background winking at everyone when Cox is talking to Carla

Great episode.
I don't think the baby will be that big of a deal. So much of the show takes place at the hospital that there won't need to be much explaining as to why they aren't showing the baby much. Jack only seems to make appearances occasionally and it doesn't seem odd or out of place.

Did no one else notice the blatant reference to Office Space?
Dr. Cox, while ignoring Eliot, decides to talk to a stapler instead. He compliments the stapler on its red color. Red staplers did not even exist prior to Office Space. The movie promted Swingline to create a line of red staplers.
For those truly ignorant, John C. McGinley was in Office Space as Bob, the consultant.

Did anybody catch how bad Christa Miller looked in the final crowd shot?
As the season started, I chalked up her declining appearance to a botox-gone-wrong misadventure. But by her increasingly swelled face, I'm beginning to suspect something more serious is afoot.
I certainly hope I'm wrong.

I noticed the shot of Christa that you're talking about. She looked rough man. Not sure what's going on there.

Question: Love your column! My question is, now that NBC has shelved Four Kings (which I actually liked), what do you think about moving Scrubs to the 8:30 pm/ET time slot that Kings occupied? Maybe they could gain the audience they deserve if they were sandwiched between Will & Grace and My Name Is Earl. — Gina
Matt Roush: I think it's a great idea — just about everyone except the NBC programmers seem to agree — but I don't see it happening, not this late in the season. NBC just threw another ball of crap at Scrubs last week, pairing it with the miserably unwatchable Teachers, and I would imagine for much of the rest of what's left of the season, NBC will continue to either double-run Earl episodes on Thursdays (way to burn out a hot new show) or replay classic Will & Grace episodes as part of the countdown to the end. Or maybe they'll find a way to sneak even more editions of Deal or No Deal across the lineup. Who knows with this network? If Scrubs survives this season, it will be no thanks to NBC.
