For me I would have to say Will and Grace really needs to go. Even though I still watch it there hasn't been so much as a chuckle from me for at least a year.
A few others I can think of are Stargate SG1, Simple Life, King of Queens, and According to Jim.

Granted the X-Files after season 5 pretty much sucked.
I forgot about the Simpsons. That's probably because I haven't watched it in about 5 years. It was stale then.

Will and Grace, absolutely.
The West Wing.
And I hate to say it, but The Simpsons. I think it's been great but this last season it's starting to feel tired like it never has before. I worry they're just trying to make 20 years just to say they did and the show's reputation will suffer as a result.

I think ER should have ended with the death of Mark Greene. They went from his great last few episodes to a goofy viral outbreak cliffhanger in a matter of one week. That was the beginning of the end of ER as a serious show. Every week is just now some stupid "biggest week ever in the ER ever" episode like Rocket Romano getting crushed by a helicopter or other silly plot device.

Opening the thread ER was what I initially thought of. While not the longest show on TV, it feels like it is. The last few seasons have been a wreck. The episodes where Mark was dying were the last of the good ones. There hasn't been one sonce. Maybe the Romano dying ones, but even those were sub par.

It might also have a little to do with Parminder Nagra and Linda Cardellini.

Simpsons - it jumped the shark the day that the show made fun of some country and when the board of tourism from that country complained, the writers apologized. The show officially became lame right then and there.
All reality shows - From the inane (Survivor) to the slutty (Bachelor / Bachelorette) and all in between. Is there anyone left that actually believes that these are "reality"? Conflict is scripted and manipulated to draw attention.
South Park - Maybe they're just in another rut, maybe it's because I've been watching the first 3 seasons on DVD lately, but the 3 episodes of this new season have all sucked.

More votes for ER, Reality TV as a whole (excluding Project Greenlight), CSI Miami. I would put Will and Grace and According to Jim, but those wore out their welcome with me after one episode apiece.
Everyone's being hard on The Simpsons and I can see the disappointment when judged against its own glory years (years 3 through 6 in particular), but it's still better than 90 percent of most other 30 minute network comedies.
