Any TV show where unexpected pregnancy complications are wrapped up tidily with a miscarrage (Moonlighting, Anything But Love, etc)
Any show that undergoes major retooling and cast changes but pretends like we won't notice

Any "Very Special" episode of 'The Facts of Life' or 'Blossom'

Tara was introduced at the beginning of Season 4. Willow "came out" 19 episodes later. In TV terms, that is a very long time (22 episodes comprise roughly 'one year' in the Buffyverse). If you in fact did not miss any of those episodes in between, I can only suspect that you failed to pick up on subtext.
I can only suspect you have not watched season four in a while. Oz and Willow are together and happy through episodes 1-5, break up at the end of episode six "Wild at Heart." I can't say for sure when Tara is first seen, but her first credit according to the episode guides I checked is on "Hush" which is episode 10. So my ability with subtext may be suspect, but you have your own problems with text.
