Why would Melfi say "calling all cars" (the title for the episode)? What significance does that have?
And why would she call him right away, as if it's such an urgent thing to say?

I'm guessing Melfi was assuming she was the only thing keeping Tony from completely spiraling out of control which is why she had the urgency in her voice telling him to try one more session. Now that he's out of therapy (for now) he's going to lose it and maybe go on a rampage, hence, calling all (police) cars. The rampage I assume will probably be the death of Johnny Sac, maybe Paulie, and I think a good chance for Carmine too.

I thought the episode was fine. It's now fairly clear that when the Sopranos was signed for a fifth year, they took the main projected plot of the expected fourth and final year and stretched it over two years.
I like dream sequences, when they're done well-- e.g., sufficiently dreamy. The car dream was really interesting; the house dream was beyond me.
I think Melfi called her doc so quickly because she feels relieved, abandoned, and sorry. I thought she might be indicating that she needed help.
Janice may be the most despicable character in the history of television, Real World included!

I thought this episode was a little slow myself but I still think it did a great job with Melfi and Tony. Who seriously thought that he would leave treatment with Melfi? It was kind of a shocker to me at least. I think things is gonna get interesting real soon in the next 2 episodes.

Couple predictions
1) Paulie goes to New York to join Johnny's family after Johnnny kills Carmine after he finds out Carmine wanted him killed earlier in the year. Johnny sends someone to Florida to kill Carmine Jr. Johnny makes Paulie his "underboss." Paulie takes lil Paulie with him. The new New york family is the Sacramoni family.
2) Tony orders Benny to kill Furio after he tells Carmela his feelings for her.
3) Tony will resume his thereapy by the season finale.
4) Tony has to kill Chris after he finds Chris still into drugs after rehab. That would end the Adriana-FBI storyline since thet wouldn't need her if Chris was dead.
I'm probably wrong but we'll see.
This week's episode was the second worse of al time next to 'Christopher.'

I was thinking about Tony's dream, the one where, He, Ralph, Gloria and Carmella are in the car driving. Two of the people in the car are dead, one at the hands of Tony and one for which Tony is not responsible but blames himself for anyway. Carmella has nothing in common with Gloria or Ralph, so what is the signifigance? Is it Tony being concerned of endangering those he cares for? I'm stupid.

Carmella is in control of the car, something that Tony isn't too happy about because Tony has control issues. That's the significance of the dream, that he's losing control.

3) Melfi- I never really like the melfi visits

Really boring episode.
It clocked in at 45 minutes without pre-views before.
The show really suffers without Christopher & Adrianna.
This season there's been hardly any Meadow and Melfi has barely been on and only in the psych office.
They need to have more with just Tony and Carmela if they're
gonna screw all the other good characters.
