Where at Netflix can ask for replacement to scratched disk?

Where at Netflix can ask for replacement to scratched disk?
I've reported it unplayable. Then I got email saying a replacement will be sent IF I selected to have replacement. But I was not offer that option, where might that be?

I'm pretty sure it gives you that option when you report the problem.

Go to Your Account and click on Report Problem. This will allow you to request a replacement disc.

A sleeve that came with my most recent Netflix selection says:

If this DVD is unplayable, get an immediate replacement DVD by visiting: www.netflix.com/replacement
