Did we ever learn the reason he was replaced?

I thought it was well established that during the scene where his character stood behind Nicole Kidman and masturbated Harvey was such a methed actor he started pulling his pud for real and accidently shot a load in her hair. She was so outraged that Kubrick had no choice but to fire Keitel. And to make sure no one ever found out, Kubrick re-wrote the script to remove this scene entirely when the role was recast. Since the production of Eyes Wide Shut was so long, there were all kinds of rumours about it back in 1997-1998, like it was going to have hard core sex scenes between Cruise and Kidman.
