Mine are pretty much already taken, but I get snagged on names a lot:
Dylan McDermott Mulroney
Michelle Williams & Carey Mulligan. I've only seen one each of their movies but I confuse the two younger, talented, blonde, Oscar hunters who are apt to do nude scenes.
Keith David/David Keith - I've seen loads of Keith David's movies and consider myself to be a big fan of his, but I can never remember which name comes first. Damn use of a first name for a last name.
And I'll add Jason Robards and James Coburn for some reason.

Michael Musto in the Village Voice once did a column on how he kept confusing, Dylan McDermott, Dermot Mulroney, D.W. Moffett and D.B. Sweeney. One of the latter two then left a nasty, homophobic message on his answering machine, giving him fodder for a whole new column. This was at least 20 years ago, when either of those actors mattered. (If they ever did.)

Another group that I confused when all three were on network TV series:
Jill Hennessy
Amy Brenneman
Kim Delaney
It didn't help that both Brenneman and Delaney were on NYPD Blue, and that Brenneman's series was called Judging Amy, always leading me to think that the actress's first name was not the same as the character's, when in fact it was.

Saturday Night Live had a recent skit about a gameshow where the contestants couldn't tell those two apart. It was actually funny, and didn't go on too long. Most SNL skits are cute for the first 30-60-90 seconds, and then the Not-Ready-For-Prime-Time players stay on too long.
