I had probably seen Excalibur about 4 times before I noticed Gabriel Byrne listed in the credits as Uther.

for some reason, the look and the name didn't connect. And i didn't know she was in it either.

I didn't recognize Judah Friedlander in The Wrestler.

I dunno if this counts but when I watched Mama Mia I couldn't shake the nagging suspicion that I'd seen Amanda Seyfried somewhere before. It wasn't until last night, while watching TBS, that I realized she was the ditz who thought her boobs could predict the weather in Mean Girls. She was also a bridesmaid in an episode of CSI: Las Vegas. I wish she still looked the way she did in that movie and that episode.

Damn, she looked good in Mean Girls.

Yup. I wish she'd go back to that fuller look. She doesn't look good all twiggy like she is now.

Not a film reference but I was watching this past week's episode of Law & Order: SVU and couldn't for the life of me place the defense attorney. I could tell from his facial features and his voice that I had just seen him in something but it didn't click until towards the end that was Giovanni Ribisi's father in Boiler Room. He's just aged quite a bit.

I couldn't recognize Vera Miles from an episode of Columbo I saw recently, the bad thing is I had just re-watched Psycho a few days before.

Al Pacino in The Godfather. Yeah, I didn't see the movie until 2008, and I'm more used to the way he looks today, so that was...a surprise, but I should have known.
