I'm betting he used a computer program to place each piece. Something similar to those programs that create pictures out of thousands of little pictures.

To make this would be easy.
Create an index color version of the image. hand pick the colors based on the primary colors of legos. Print an enlarged version, but turn off interpolation when you resize. so each pixel ends up being a square the size of a lego piece.
apply color legos to same color on paper.
I'm going to try it... Where do you buy 1x1 pieces?

I haven't checked this site, but I've heard http://shop.lego.com and www.bricklink.com mentioned somewhere.

I see where you're going with this, and it makes sense, but how would you make an "index color version" of the image? (what software, etc).

And I'm not as impressed now (because it's computer-aided) but it would still take quite a bit of time and patience to hand-place more than 4,500 Lego pieces in the correct order.

Actually, I just took a look at that professional Lego builder's site above, and apparently even he uses software when he creates mosaics (I was under the impression that the Pro's did it freehand)

It sounds a bit more complicated than I thought. Here's a link from his site:
