I asked Quentin Tarantino tonight. The guy has a pretty encyclopedic knowledge of films, and his response was, "good question...what've you come up with so far?" I told him "The Cabinet of Dr. Caligari," and his response was, "Yeah, I guess that is a twist ending, and probably one of the first." If that man doesn't know, I guess there's no definitive answer to be found.
