Since Keynote has taken over they're not as quick with the GCs as Vividence was. 7 days after you've done the survey I find you have to bug them to get them to send the certificates out.

Then click awards in the top right.
If you see it listed, there's an option to click to have them send the code to your e-mail again if you haven't received it.
If you don't see it (which I don't yet) go back and click "participation history" and make sure it shows up there (Keynote Study (9678) is what to look for.
If it's there then just be patient, since they've switched to keynote they've been slow about getting the reward codes out, and I had to get my last one resent as I never got it the first time.
