I bought the PS3 online from Sony Style. Do i include the Packing slip or the e-mail confirmation with the order form. Thanks!!

packing slip and the UPC is well, the UPC. Just cut the entire thing out and send it in. It's the unique serial that's near the UPC they are most interested in, I believe.

If you haven't sent it in yet, upon calling I was told the upc was the POS Bar
Code and should start with 11719.

Is this 5 free Blu-ray order form only available to customers who bought the PS3 at Sony Style?

The offer was good for anywhere that you bought it. You had to buy it by January 31 and you must have it postmarked by February 29.
