There were/are multiple HD DVD 5 free MIR forms out there, but I've only been able to find the following form for the BluRay's and am a bit underwhelmed by this particular selection... any others out there other than THIS one:

There was only one HD-DVD MIR form at a given time (for a total of 2). I'm afraid you're out of luck. There's only one BR MIR as well.
