I just got my HD-DVD player through amazon and I forgot to print off the rebate form for the 5 free DVDs. I bought it last week and now I can't find the rebate form from last week, I can only find the new rebate form for those who bought their player as of October 1st. Does any one have a scan of the old one or know where I can get it?

It's been just over two months since I sent my order form for my five free discs and I've yet to receive anything. I tried calling the 800 number but all I got was a fast busy. I see nothing on the fine print that says Puerto Rico is excluded so I don't see why the number doesn't work. Is there another number or an e-mail address I can try?

I haven't gotten mine either. They say 8-10 weeks so I would wait a little bit longer.

I got mine after about 12 weeks. Sent the card off mid-June and received them mid-September.
