How often does it occur from discs manufactured in 2010 to present?

What are the symptoms of it, and what does it look like?

Too soon to tell for ones made after 2010- I've had a few from 1998-99 go bad though, one turned a kind of dark color but the others looked the same but would not play anymore- when trying to play them the player just says "NO DISC", while others might play a bit but glitch at some point during the movie. Many HD-DVDs (made from 2006-2008) developed that problem as well.
