Never bought VHS, mostly took advantage of the rental boom of the mid to late 80's and into the 90's. When I did buy them thy were for my baby sister. So it was probably Barney and some Disney classics.

VHS: Pepe Le Pew's Skunk Tales (cartoon compilation from WB) - 1988
DVD: The Abyss (2-disc) and Weird Al: The Videos (Image) - Sept. 2000.
Never got into CED, LD, or Beta.
My first BluRay, when I can afford to get into the format, will be 2001: A Space Odyssey.

5 Free-by-mail... but I don't remember what those were... and I think I exchanged a lot of them rather than keep them.

VHS
I had tons when I was a kid, but the first I bought for myself was Batman Forever back in October '95. That's when my collection really began.
DVD
The Matrix
The Avengers
Analyze This
Ghostbusters
Blade Runner: The Director's Cut
Godzilla (1998)
I wouldn't be surprised if my first blu-ray ended up being Quantum of Solace.

I don't know how people remember things like this. These are best guesses for me.
Betamax - never even touched a betamax in my life
VHS - a Disney film probably
LaserDisc - never owned, rented, or watched a LD
edit to update on 12/3/10: I found a free laser disc player in late 2008, and just recently bought my first three lasers, Dead Again, The Wizard of Oz, and Beauty and the Beast
DVD - Star Trek: First Contact ?
HD DVD - freebie NiN sampler at Best Buy; first real title was the free King Kong in my 360 add-on; first paid for title was the 5 disc Blade Runner set
BluRay - freebie Spider-man preview at Best Buy; first real title free Deja Vu; first paid for title Close Encounters of the Third Kind Anniversary set

Beta - The Muppet Movie. My Dad still keeps a Betamax player hooked up. No clue why but he prefers recording the few shows he watches on it.
VHS - Raiders of the Lost Ark. This one was a little hard to remember.
LD - Never owned a player or movies but I often lusted after the awesome edition of The Frighteners than LD owners had the opportunity to own.
DVD - Before I picked my, highly overpriced, player up way back in 97 I bought Spawn, Grumpy Old Men, Grumpier Old Men and My Best Friend's Wedding. I always have a habit of buying the media before I buy something I can play it on.
HD DVD - I was tempted a few times but never jumped on it.
Blu-Ray - PS3 came with Spiderman 3. I then IMMEDIATELY ordered the new Blade Runner 5 disc edition.

My first VHS purchase (back in the day when VHS movies cost 80.00 brand new!) was Wings of Desire. Needless to say, I didn't buy any other VHS tapes when they were that high priced, and I REALLY wanted Wings of Desire to watch again and again.
I consider my first DVD purchase Mallrats, though I think I purchased four DVDs on my first DVD purchase but the whole motivation to buy was Mallrats, and that was the first one I put in my amazon.com cart, so I consider it my first DVD.
And I've never seen any other format.

VHS and standard DVD are the only home video formats I've owned.
The first VHS was Raiders of the Lost Ark (received as a gift along with my first VCR).
First DVD was a boxed set of Robert DeNiro movies, given to me along with my first player by my wife (then girlfriend).
That first player, by the way, she ordered from 800.com. Who else remembers them?

I'm old enough to have potentially owned a VHS or Laserdisc collection, but actually didn't start watching movies (literally, any movies) until my 20s. So the very first purchases - the stuff I picked up at the same time as their respective players - since then...
DVD
Session 9
Seven: 2-disc
Unbreakable
HD-DVD
The Matrix collection (only got into HD because of this $20 pricing error at Circuit City!)
Blu-ray

I've only owned titles on VHS, DVD, and Blu-ray.
I really don't remember the first few titles I bought on VHS (I don't own that many titles on VHS quite frankly), but the "Star Wars" movies, "The Lion King", and "A Goofy Movie" (don't laugh, I loved it) are the ones that got watched the most.
With 572 DVDs; I really can't remember my very first title, but "The Simpsons" seasons 1-3, "The Green Mile", "Shrek", "Saving Private Ryan", and "We Were Soldiers" are some of the first DVDs I bought (I remember those because they were bought from CH).
Since I only started buying BDs about a month ago; it's pretty easy to tell you my first few titles. "Die Hard", "30 Days of Night", "Die Hard 2 - Die Harder", "No Country For Old Men", "Die Hard With a Vengeance", and "I Am Legend".
