There's not really a great way to do it at home. There is this product which works for some...:

Skip Doctor

These things basically work by sanding off some plastic until the disc is optically readable again. (For these home products - the discs usually look pretty bad afterwards though.)

I would recommend taking it to some local rental stores or used movie / video game shops and see if they have a professional "re-surfacing" service. They usually work the same way (by sanding off some plastic) but they do it better and the disc usually looks nearly new after it's done. Places where I live charge $2-$3 per disc.

I used to work at an EB Games and picked up a Mad Catz motorized disc repair kit at the time. I mostly used it for fixing used Xbox and Ps2 games. I've had it almost 2 years now and it has worked on everything I have used it on. The only problem is that the extra pads are not sold in stores you have to order them from Madcatz. I know there is an identical looking product by Memorex and its extra pads are sold in stores (they don't fit the Mad Catz unit), but I have not used that one.
