I dunno if it's mentioned but I have to give a vote for Sonata Arctica's For The Sake of Revenge. It's a 90+ minute video of their concert in Japan and it includes a 70 minute CD of the same concert (for around $15.99).

Elvis: The 1968 Comeback Special (Three-DVD Deluxe Set). Not "technically" a concert, but the best in my collection, has TONS of outtakes on it!
