I once saw a dab of green paint on the playable side of the DVD, like when they printed the DVD art on to the DVD itself, maybe some of the ink/paint splashed over onto the other side. It rendered the DVD unwatchable, of course. That's literally the weirdest thing I've seen on a DVD.

If you are specifically wanting to talk about the strangest disclaimers, I don't have a screen shot, but I have a region 2 DVD (I think it's Help, I'm A Fish but I am not positive) that in the disclaimer at the beginning, one of the places it lists prohibiting the showing of this movie is on oil rigs.
