Does any place sell extra pages that can go into a DVD case?

Depends on who made the case. check at the manufacturers site.

Are there no generic pages? I'm not sure who the case manufacturer was.

But many of the manufactured binders use a different hole pattern. Thats why you may need to deal with the manufacturer. In addition, alot of them do not allow addition of pages.

I don't mean for a notebook but for a 14mm plastic DVD cases that most movies come in.
