How can I prevent damaged dvd cases

Please tell me how I can prevent my cases from hole's and dents Im seeing this more and more in my collection and don't understand why this is happening. Im very careful with my collection but can't seem to prevent this from happening

How do you store them currently......I take all my dvd's out of their cases and store them away in a box. Then I put the discs in a seperate sleeved storage unit. That way i get to watch the movies when ever, they take up less room next to the TV and the oringal boxes remain mint!
