My Dinner with Andre.
Washed out colors, pan and scan, colors that change from shot to shot...
I think they used an old VHS copy as the master.
I still bought it though... grrr.

The Monty Python And The Holy Grail subtitle problem
It kept me from owning it for 2 years now since my DVD player wasn't compatible with it. Supposedly they've fixed it, but they forget to differentiate the new version with some kind of marking/coding on the packaging to keep people like me from picking up the bogus one again. Plus supposedly even the fixed version is missing the Swedish subtitle. It p*sses me off because the film is easily in my top 5 favorite films of all time. I returned the DVD back to the store a week after it's release day.
Yes I'm aware that you can mail in the defective one for a fixed one, but unfortunately they make you pay for the shipping and there's no guarantee that it's not going to be lost in their mail room, never to be seen again. Columbia/Tri-Star wasn't exactly out in the open about the problem so it was very iffy if they were going to do anything at all.
Anyhoo... I've recently ordered the Collector's Boxed Edition with the thought process of "being that it's a brand new release, it should for sure come with the fixed one!"
Right?
(And from the problems the new Meaning Of Life disc seems to be having, it looks like I'll have another 2 more years before another Python disc gets put into my collection)

Classic Media's "The Ultimate Godzilla DVD Collection." Four out of the five movies in the box-set were the same as what was in Simitar's oop box set. Plus, Simitar's discs are better! Only saving grace is that "Rodan" is on DVD now.

Since "The Sting" was already mentioned, I'll add my own two cents (or crimes).
1. Many people here are not aware that the Warner DVDs released in the first batch of this format, in 1998-99 (like "Frantic" and "Spies Like Us") are only available full-frame in North America, while the same DVDs, with exactly the same packaging, are available in R2 in OAR. Nice thinking, Warner!
2. The "Zorro" 1970s film, starring Alain Delon, was butchered beyond recognition by cheap bastards (a la Madacy) who reduced a 130-minute movie to barely 90 minutes.
WHY?

What dumbass thought that that needed to be included? Yeah, much better choice than a commentary track. [SARCASM]Let's all take just a moment to congratulate Universal Home Video on their unparalleled excellence in DVD production...nice work boys![/SARCASM]

It wasn't a thought-provoking, or even good feature, but hey...there's a logic there.
But Universal is a terrible company in the DVD market, I agree.

"Chariots of fire" is another one of the WB movies released P&S by Warner, like my previous post indicates. If you look at the R2 versions specs, it's exactly the same, except that the R2 version is widescreen.

Also, no Kagemusha on dvd...am I right? That movie might be even better than Ran.
Others:
Bertolucci's "The Last Emperor": awful picture and sound.
My Neighbor Totoro: Full screen only, English only. Probably my second favorite Miyazaki ruined by the geniuses at Fox.
Miramax: they're finally coming around, but all of their first gen titles blow.
Spaceballs: misframed so badly, it actually ruins the "nosejob" joke.
Diva (both releases): terrific French action movie (one of my favorites), I bought it twice and both discs kinda blow (the second one is a little better, though). Worst double dip ever, at least do a good job second time around.
Doors SE: see the comments above on Diva
