I bought mine at Costco (another Membership Warehouse) in Feb 2003 and am positive it has an insert. I'll double check when I get home.
EDIT: Yes, it does have an insert. Single page with Chapter list on one side and list of extras on the other side.

MIne has one as well. I bought it when it was first released.
If you call Buena Vista HOme Entertainment CS (800) 723-4763 and explain to them that yours came without it they will send you a new one. I bought a few PVDVD's from BBV and they all came without inserts. I called and requested them - they sent them right out.
