LOTR Extended Missing Booklet?

LOTR Extended Missing Booklet?
I searched for details on this but couldn't seem to find anything anywhere... so here goes:
Am I the only person who upon opening their LOTR Extended DVD set found myself missing the advertised (on the GLUED on insert) 12 page booklet? I got a two page chapter listing booklet stuffed into the left side of my set with no booklet.
I know I have a canadian version because the "Maritime Rating" system stamps are clearly on the glued on insert itself.
Anyone?
Furthermore - does anyone have any way I can get into contact with New Line Customer Service to try an rectify this?

Mine came with the booklet.
I don't know if the Canadian version came with one.
There's a forum that contains customer service numbers. You may have luck trying one of them.
Personally, I'd try the store I bought it from first. Explain your situation and take/send it back to exchange it for another copy.

If the booklet is EE exclusive (damn, have to both of my versions), then you will not have the chance to steal it from blockbuster case.... that is if they haven't already been stolen.
