This is one awesome dvd! One of my favorites. Picture and sound are great, and the extras are spectacular. The documentary on the making of the film runs over 90 minutes, and is the most entertaining and informative extra I have yet to encounter on dvd. There is also a commentary track with Terry Gilliam and someone else (can't remember who)

12 Monkey's is anamorphic, and it looks great. The sound is very good. But as a movie, it's a must own. I'm not a big fan of Gilliam (Fisher King is one of my most hated films and I have still never sat through Brazil without dozing off), but this film is a classic. I'm a sucker for time travel stories anyway, but this one is done very well. I didn't even mind the overly cute and eccentric Gilliam touches that made my skin crawl in Fisher King.
As far as extras, this one has the longest documentary I've seen on a DVD, and the commentary track has some interesting insights into the making of the film (the anecdote about which kid to use is fascinating). The making even has chapters -- many making of's don't have chapter breaks or even a clock.
A great flick, eminently reviewable with lots of extras. Well worth the money.
