Good until 1/6/06, see specially marked packages. You need the UPC code from the Minute Maid, the offer form(on specially marked packages of Minute Maid) and the proof of purchase tab or UPC code from March of the Penguins, as well as the receipt from the DVD purchase.
Yes, it means that you would have to cut the DVD package as both the UPC code and the pops are on the outside of the DVD. The pops are right on top of the UPC code, so it is possible to cut one of them without defacing the packaging too much.
