I recently purchased the Dead Zone, seasons 1 and 2, and on season 2 it says there's a $10 rebate if you buy both seasons. There's no rebate form in the box though. Does anyone know anymore information about this rebate?

My set didn't have the rebate form either. Could someone email me a scanned copy if you have it?

I just got a color pdf of the rebate form, anyone that needs it feel free to email me. I sent it out to anyone who's email was in this thread.

This rebate arrives very quickly. Mine took less than 4 weeks.

I also sent in copies of the UPCs with a note saying that there weren't any PoP tabs and cutting the UPC is pretty difficult since it's part of the DVD Packaging.

If the coupon is still valid I would appreciate the pdf.

I just mailed away a black + white copy of the form, along with copies of the UPC's. All that copying and printing was $10 work alone.

Well I sent the copy of the rebate form with the scan of my UPCs and the packing slip and today I get a letter from them in the mail that says:

You are receiving this notification because you have not complied with the terms of the offer due to the following reasons:
Required documentation was not included in submission - i.e. Rebate Form, UPC Code, Store Receipt, Invoice.

Original rebate form. Not Received
Original or copy of receipt, invoice or packing slip Received
Much purchase both items needed for rebate. Received

So does anyone have an extra rebate form they can send me? I'll gladly paypal you for the postage. My email is: [email protected]
