Warner Rebate Form???

Warner Rebate Form???
So I purchased Flintstones Season 1 and Jonny Quest from Costco and on the box it says get $10 back in mail if you buy 2 select Warner animation titles and $15 back if you get 3 (On those long Costco boxes it said this), it also said see details inside. So I open it up and there is no paper or anything. So I assume for sure its in the box sets. So I open up the flintstones set, NOTHING...then I go okay, 1 more chance with the Jonny Quest......NOTHING
Now what do I do? The box says see details inside and yet there is jack!
I plan on buying 4 more so thats $30 total, someone help me out please

Oh alright, so I just print that up and fill it out?
And for the box sets, I bought them at Costco and on the receipt it just says "DVD/Hanna #2", thats what it says for the Flintstones season 2, it doesnt say Flintstones on it...will they take it still?
And how the heck am I going to cut out the UPC since the cover is plastic and you cant rip through it, you have to cut like all the way across.

Arghhh! I ordered Batman Vol 2 and Superman Vol 1 from Amazon on Dec 28, 2004. Both sets arrived today (Jan 27 2005). But lo and behold: there is no rebate form to be found. Checked both sets, neither one has the form. Looks like my only choice is to print out the form from the link provided above... but... it clearly states you must send "this original form, no copies accepted." Will they honor my print-out version as an original? Damien666Omen-- did you send in a print out using the link above and did they accept it? I wonder if Amazon would be able to mail me an original rebate form? Did anyone else receive either of these sets from Amazon, and if so did you receive the rebate form with them?

I also received no rebate form in either of my Batman sets. I just got off the phone with Warner Brothers Home Video customer service and explained that I got them both from Amazon and asked if a copy of the form would be accepted since neither of my packages had forms included. I was told "originals only", but the CSR apologized for the missing forms, took down my info, and said they would mail me out a rebate form ASAP this AM and that as long as I get it in by 2/22/05...we will be good to go. Really nice lady who was plenty helpful. Even said she would be sending along an email to her surpervisors that I got them from Amazon and that they may want to check into why this would be happening. If anyone is in the same boat...you may want to give em a call and tell em your story.

The phone number for the Warner Rebates is 1-866-488-4640 (this comes from the pdf form available on the Amazon link). I called this number on Monday and spoke with Dori. I explained to her that my packages from Amazon did not contain the rebate form. She said that I could find the form on-line and print it out. I expressed my concern that a print-out might be mistaken for a copy, but Dori said that a print-out would be accepted. She even put me on hold for a moment to verify this with her manager. Her manager confirmed that a print-out would be acceptable. So... I printed out the rebate form off of the Amazon link (using a color printer, to be extra safe). I included a note stating that I spoke with Dori on Feb 1 and she told me that a print-out form off of the internet would be accepted. I mailed my stuff on Tuesday (Tab D's included), so it is too late for me to consider having a rebate form mailed to me. But, according to Dori, it should work out. We'll see. If it doesn't, I will call back and complain big time.

I just got my $15 check. YIPPEE!!! A word of caution for you all regarding these animation sets though. I dropped one, and the plastic case SHATTERED into tiny bits and pieces. They may look cool, but they can easily get severely damaged...

The Batman 2 and Superman 1 DVD sets I got from Amazon had no rebate forms (or any other inserts for that matter) either.
I already sent in the $10 rebate back in November when I bought Batman 1 and Looney Tunes Golden Collection 2 so I bet I can't send in for this second one for two different sets, right?
