Dead Zone rebate question

I'm about to send off for a $10 rebate, but the form says to include the original proof-of-purchase tabs found on the back of the video package. Thing is i can't seem to locate this. Is this refering to Video tapes, or is the tabs hidden away?

It is on the DVD.....if you cut carefully you can still play the disk.*

I remember someone stating in an older thread that they made a photocopy of the UPC from the box and that they did receive the rebate.

That's exactly what I did, along with a little explanation on the photocopy. They sent it rather promptly too if I remember correctly.
